Creates a dedicated public/single-opt-in list ('auto.levkin.ca leads', id 6) in Listmonk and hardcodes its UUID into the subscribe form, replacing the REPLACE_WITH_LIST_UUID placeholder. Smoke-tested against the live /api/public/subscription endpoint.
